Rapamycin is the most interesting longevity compound in human medicine and the one most people are too scared to actually use. I’m not. I’ve been running it on and off for over four years now, watching my own labs, watching my training, watching how the protocol interacts with everything else I’m doing.

Here’s what I’ve learned. And what I’d tell my younger brother, my training partners, and the guys in my circle considering this protocol.

What Rapamycin Actually Is

Rapamycin is a drug originally isolated from a soil bacterium found on Easter Island — Rapa Nui, hence the name. It was developed and approved as an immunosuppressant for organ transplant patients, where it’s used at chronic high doses to prevent rejection.

But the longevity world cares about it for a different reason. Rapamycin is the most reliable lifespan-extending compound ever tested in mice. Not by a little. By a lot. Across multiple labs, multiple strains, multiple dosing protocols, it consistently extends both median and maximum lifespan by 10-25% even when started in middle-aged or old animals.

The mechanism is mTOR inhibition. mTOR is a master regulatory pathway that decides whether your cells are in growth/anabolism mode or in maintenance/autophagy mode. Chronic over-activation of mTOR — which is what happens with constant high protein, frequent eating, and modern lifestyle — drives accelerated aging at the cellular level.

Rapamycin pulses mTOR off. That gives your cells time to do the cleanup work — autophagy, damaged protein clearance, dysfunctional mitochondria recycling — that they can’t do when growth signals are constantly on.

Why The Pulse Matters

This is where most people get it wrong. Rapamycin at chronic daily doses, the way it’s used in transplant medicine, has a long list of side effects: immune suppression, glucose dysregulation, mouth sores, impaired wound healing, dyslipidemia.

The longevity protocol is fundamentally different. It uses weekly or even less frequent dosing. The goal is intermittent mTOR inhibition — enough to trigger the autophagy and maintenance pathways, but not enough to suppress immune function or cause the chronic-use side effects.

This is the protocol that’s been pushed by Bryan Johnson, Peter Attia, and most of the longevity-focused MD community. And it’s the only protocol I’d recommend anyone consider running for anti-aging purposes.

My Actual Protocol

I’m going to share what I actually run, with the caveat that everyone’s biochemistry is different and you should not blindly copy this without monitoring your own bloodwork.

Current protocol (years 3-4):

  • 5mg rapamycin (sirolimus) once weekly
  • Taken in the morning, on an empty stomach
  • Grapefruit juice protocol if I want to bump bioavailability (more on this below)
  • Continuous, no scheduled breaks

Year 1 protocol (starting out):

  • 2mg weekly for the first 4 weeks
  • 3mg weekly for the next 4 weeks
  • 5mg weekly thereafter
  • Pause if I caught any infection
  • Pause around any heavy training cycles or competitions

Why I started low: The half-life of rapamycin is around 60 hours. At weekly dosing, you’re getting incomplete washout between doses. Starting at full dose with that pharmacokinetic profile is asking for surprise side effects. Building up gives you signal on how your body handles it.

The Grapefruit Juice Hack

Rapamycin is metabolized by CYP3A4 in the liver and gut. Grapefruit juice contains furanocoumarins that inhibit CYP3A4. Taking rapamycin with grapefruit juice can increase its bioavailability by 30-100% depending on the timing.

The protocol some people run is 8oz of grapefruit juice 30-60 minutes before the rapamycin dose. This lets you achieve higher peak blood levels from a smaller pill dose. The risk is dose variability — if you don’t consistently use the same protocol, your effective dose swings.

I don’t use the grapefruit hack consistently. I run 5mg straight. But I know guys getting solid results at 3mg + grapefruit who basically equal a 5mg dose without the cost of higher physical pill consumption.

What My Bloodwork Shows

Four years of quarterly comprehensive panels. Here’s what’s stayed clean on long-term weekly rapamycin:

  • Liver enzymes (AST, ALT, GGT) — all in normal ranges, no creeping
  • Kidney function (creatinine, eGFR, cystatin C) — stable
  • Lipid panel — small uptick in LDL by maybe 10-15 points (consistent with what’s reported in human studies), HDL stable, triglycerides stable
  • Fasting glucose and HbA1c — stable, no insulin resistance signal
  • Comprehensive metabolic panel — clean
  • Testosterone, free testosterone, estradiol — stable (rapamycin doesn’t directly hit the HPG axis at weekly doses)
  • Complete blood count — neutrophils and lymphocytes in normal range, no immune suppression signal

What I monitor more closely:

  • Fasting insulin — has stayed in low single digits
  • Inflammation markers (hsCRP, IL-6) — actually trending lower over years
  • Apolipoprotein B — slight increase consistent with LDL change, addressed by my baseline ApoB-lowering protocol

Subjective Effects

I’ll be straight — rapamycin doesn’t feel like much day to day. This isn’t a peptide where you notice acute effects in your training session or your focus.

What I’ve noticed over years:

  • Recovery from training feels marginally better, particularly from heavy loading phases
  • Skin texture has held up better than peers in my age cohort
  • General sense of energy and resilience is high
  • Less of the “creep” of low-grade chronic inflammation that hits most guys past 40
  • Faster bounce-back from minor illnesses

What I haven’t noticed:

  • No magic strength gain
  • No dramatic body composition change
  • No fountain-of-youth subjective feeling

The gains are subtle and accumulate. That’s the nature of this kind of intervention. It’s not a stimulant. It’s a maintenance protocol for cellular machinery.

The Real Risks To Take Seriously

I’m not going to pretend this is risk-free. Things to actually watch for:

Wound healing. mTOR inhibition can slow wound healing. If I’m getting any procedure — even minor — I time the procedure for 7-10 days after my last rapamycin dose. Heavy training that produces a lot of microtrauma is fine on weekly dosing, but I’d pause for major surgery.

Infections. Even weekly dosing modulates immune function slightly. If I’m sick, I skip the next dose. If I’m traveling somewhere with infection risk, I plan around it.

Mouth sores. The most common side effect at higher doses. I haven’t had them at 5mg weekly. They show up more frequently at 8mg+ weekly or daily dosing.

Insulin resistance signal. Some users develop transient insulin resistance. This is more pronounced with daily dosing. Monitor fasting insulin and HbA1c quarterly.

Drug interactions. Rapamycin interacts with a long list of drugs through CYP3A4. Antibiotics, antifungals, certain blood pressure meds, statins, anti-seizure meds. If you’re on anything else, you need a pharmacist or doctor who actually understands rapamycin to vet the combinations.

Sourcing and Cost

In the US, rapamycin (sirolimus) is available by prescription. There are longevity-focused MDs who will prescribe it off-label for anti-aging purposes — Peter Attia’s clinic, AgelessRx, and others.

Outside the US, generic sirolimus is widely available and significantly cheaper. India has high-quality generic manufacturers. I source from established compounding pharmacies and overseas pharmacies with verified manufacturing standards.

Cost runs roughly $1-3 per 5mg dose depending on source. Weekly dosing means roughly $50-150/year in compound cost. Cheap insurance against accelerated aging if the mechanism holds up.

What I Stack With It

Rapamycin doesn’t need much to do its work, but the broader anti-aging protocol I run includes:

  • Metformin — different mechanism, complementary metabolic effects
  • NAD+ precursors (NMN or NR) for the cellular energy side
  • Statin or bempedoic acid for ApoB management (the lipid side of rapamycin needs addressing if your baseline isn’t already optimized)
  • TRT-optimized testosterone protocol for hormonal foundation
  • Comprehensive peptide stack for repair and recovery

The stack is interlocking. Rapamycin handles the cellular maintenance pathway. Metformin handles the metabolic pathway. NAD+ handles the energetic pathway. Peptides handle the repair pathway. Who Shouldn’t Run This

I’m comfortable running rapamycin. I know what I’m doing. I have my labs tracked, I have a longevity-focused MD I check in with, I have years of accumulated data on my own response.

Don’t run rapamycin if:

  • You don’t have access to regular bloodwork monitoring
  • You’re not willing to actually monitor your response
  • You’re on multiple prescription meds with drug interaction risk
  • You’re a competitive athlete in any sport that tests (rapamycin isn’t typically tested but it shows up on some panels)
  • You’re trying to gain muscle aggressively (mTOR inhibition slightly blunts the anabolic response — small effect at weekly dosing but real)
  • You’re under 35 and don’t have a specific reason — the longevity benefit applies most strongly in middle and older age groups
  • You have active cancer or recent cancer history
  • You’re trying to conceive (rapamycin has unclear effects on fertility, especially in women)

Bottom Line

Rapamycin is the most evidence-backed longevity intervention available. The animal data is overwhelming. The human data is still accumulating, but the safety profile at weekly dosing in the longevity community is strong.

If you’re past 40, doing the basics well, want to add a serious tool to your anti-aging protocol, and are willing to do the monitoring work — this is the one I’d point you at. If you’re not willing to do the work, leave it alone and stick to lifestyle interventions.

I’ve been on weekly rapamycin for four years. My labs are clean. My body composition is dialed. My training output is high. I feel and function better than I did in my early thirties.
